Background

María Fernanda, a 65-year-old homemaker, lives on the outskirts of Medellín. Her life revolves around caring for her grandchildren and cooking, often preparing traditional Colombian dishes like arepas for her family. Though she has battled hypertension for years, she never prioritized regular check-ups due to financial constraints and limited access to transportation.

Presentation of Symptoms

María Fernanda began noticing swelling in her feet and ankles, which worsened by the end of the day. She also felt increasingly tired, even after simple tasks like sweeping the patio. One night, she woke up with severe shortness of breath, prompting her family to rush her to the hospital.

Key Symptoms

Edema (swelling) in the feet and ankles

Extreme fatigue

Shortness of breath, especially when lying down

Difficulty performing daily activities

Persistent cough, sometimes with frothy mucus

Diagnosis & Treatment

A chest X-ray revealed fluid accumulation in her lungs, and an echocardiogram showed her heart was not pumping blood efficiently. María Fernanda was diagnosed with congestive heart failure, caused by years of uncontrolled hypertension that damaged her heart.

Treatment Plan

Medications: She was prescribed diuretics (to reduce fluid overload), ACE inhibitors, and beta-blockers.

Lifestyle Changes: Her doctor recommended a low-sodium diet and fluid restriction.

Rehabilitation: She was encouraged to participate in a cardiac rehabilitation program to regain strength and learn to manage her condition.

Complications

Limited access to regular medical care and medications due to financial challenges complicated the management of her condition. Additionally, María Fernanda’s initial reluctance to change her traditional diet delayed her symptom improvement.
