When Hematuria and Dysuria Lead to a Surprising Diagnosis

- Diagnosis: Hematuria and dysuria caused by a bladder tumor
Background:
Javier Morales is a hardworking construction worker who often neglects his health due to the demands of his job. For several months, Javier experienced occasional burning during urination and blood in his urine, but he assumed it was related to dehydration or his physically demanding work. When his symptoms worsened, he sought medical help.
Presentation of Symptoms:
Javier arrived at a local clinic reporting severe burning pain during urination and persistent blood in his urine. He also mentioned fatigue and mild lower back pain but denied fever or recent illness.

- Blood in the urine (hematuria)
- Painful urination (dysuria)

Diagnosis & Treatment:
Initial tests, including a urinalysis, confirmed the presence of blood and abnormal cells in his urine. An ultrasound and cystoscopy revealed a small bladder tumor. A biopsy confirmed the tumor was early-stage bladder cancer.
Treatment Plan:
- Surgical removal of the tumor through transurethral resection.
- Close monitoring with regular cystoscopies to check for recurrence.
- Counseling on reducing risk factors, including smoking cessation and proper hydration.
Complications:
Delaying treatment could have allowed the tumor to grow, potentially invading the bladder muscle or spreading to other organs. Javier’s fatigue was linked to anemia caused by persistent blood loss, further complicating his recovery.
Preventative Measures:
- Educating about the importance of addressing hematuria promptly.
- Avoiding bladder irritants, such as tobacco and certain chemicals.
- Encouraging regular medical checkups, especially for individuals in high-risk professions.
Possible Outcomes:
